This is contributed by Dr Bowen as part of improving how to setup the Procedure Orders, specifically for Labs and the labcorp LEN. It should be generally useful. Comments?

Tables include adds to proc_type, list_options and users (address book)

    hey,
    Would be nice to work these into database.sql file (a couple simple copy/paste; wouldn't waste time doing on the upgrade script yet) and include in jwallace's next lab code revision for review, since that code is getting close to commit. Also interested in Rod's input (ie. is Sam using these tables the way he envisioned).

    We were (and aren't) sure if this should be part of the default install or an Optional sql data set that could be loaded by the user if desired. I'd be OK either way, but feed back is surely needed. (like idc9, cpt, ippf configs, etc..)
