#38 Billing Edits/Deleting/Invoice Notes

This adds the ability to edit/delete eob transactions. Also adds the ability to add invoice billing notes.

Your patch is basically re-writing the entire file. Please have the patch file only replace pertinent parts of file. Also, if possible, please base on the most recent cvs(development tip) or git(master branch) code.
