Depending on what AF32 means ... you can use OpalManager::SetMediaTypeOfService() to set the DiffServ bit in the IP header, which used to be called "type of server", hence the name.

But, I would recommend that you use OpalManager::SetMediaQoS() instead. See PIPSocket::QoS structure. Unless you want to do something unusual, this will set the DifServ bits to the correct values. And if you use later versions of Windows you must do it this way, as Windows has a nasty habit of ignoring the IP_TOS socket option.
