#450 OPAL doesn't end calls on read timeout

I have IVR service that establishes calls and plays music until other side is disconnected.
But when remote side crashes OPAL doesn't release call for it, because TX stream is alive and OPAL successfully sends packets to remote side.

    It is likely this requires implementation of RFC 4028 "Session Timers". As no one has funded this, I do not know when it might be implemented.

    Note, if a remote system does not send media for a configurable time, the call is cleared. Also if it cannot receive the sent media and ICMP messages are returned, then the call is also cleared.

    There were some bugs fixed in this area, please try latest Eridani.
