I strugled with this problem on my Lubuntu 16.04 with Libreoffice 5.1.3.2 for a couple of hours. The problem was that I had installed only libreoffice-writer and libreoffice-draw is also needed for Ooolilypond to work properly. Just installed libreoffice-draw and problem solved.
