I have had the same problem and solved it yesterday ;). I'm running omail on debian sarge with apachache 1.3. After upgrading from woody to sarge the problem you discribed occured. I found out that the problem has to do with sudo. After trying some different packages I found out that old debian packages like sudo_1.6.6-1.3_i386.deb are working fine, but the new stable package doesen't. I also tried the lastest sudo from source and it worked fine. I don't know your system but maybe this helps...
