Details:

Headline: add rename-reference command
Description: use findDeclaration and findReference to find the declaration and all references to a variable in order to rename it.  Find all references first (and maybe show # of refs to user) and then prompt for new name (so we don't make user input, and then err out if cursor position is bad)
