Xamine caches dialog widgets. If these are destroyed
by the user there is no catch of that fact and Xamine
will crash next time the cached dialog is reused.

Dialogs now have on destroy callbacks established to null out
the pointer to the cached dialog when closed in this manner.
The next attempt to use the dialog will therefore create a new
instance, rather than re-use a pointer to a destroyed dialog.
