#7 Allow plain text titles for notes

Instead of forcing users to supply a wiki name
(mixed-case name like ThisIsAWikiPage), allow them to
supply a plain text string with spaces, etc.

The system should automatically generate the wiki name.
Use JavaScript to dynamically generate the wiki name
from the title. User should be allowed to supply a wiki
name in a separate text box if s/he so chooses to.
