The formmail script is working (that is, the form data gets sent to the recipient) but I get a server error message after submitting the form and I can't figure out how to continue on to a "thank you" page.

What's the error message? If it's a generic HTTP/500 from the web
server, the web server should be logging a more specific message
somewhere; find and report that.
What OS? What web server? What version of OS, web server, and formmail?
How do you have formmail configured current? Consider posting an
extract of the relevant parts of your script config section and/or

Both look basically okay to me; additionally, the script config
section passes a Perl syntax check.

The best source of information will likely be the web server's error logs.
You can also try enabling debugging (set "DEBUGGING = 1"); that may
reveal more information for some kinds of problems.
