I have a custom toolbar I created in VS2005, and have imported and exported through all versions of VS up to VS2012 and it has always worked fine.
I started using VSIP 1.5.2 to inject VS2010 icons into VS2012 and installed NiceVS version 0.8.1.1 to make VS2012 GUI tolerable, and toolbar was fine.
VS2012 prompted me that there was an upgrade to NiceVS, and I let it install it. Got a new popup window from it and I chose to Fix Toolbars and it took a while but eventually gave me more nice icons on my custom toolbar and I thought all was well.
But after I exited VS and restarted it my custom toolbar was gone (not just hidden), and the defaults were shown.
I re-imported my saved settings, and got it back.
Close VS and restart and it is gone again!
Uninstalled NiceVS completely and have same behavior included the still nice looking icons!
I went back to VSIP and re-injected the VS2010 icons, re-installed NiceVS 0.8.1.1, and reimported my settings and my toolbar is back, and I can close and open VS2012 without problem.
So I suspect that something the new NiceVS 0.9 "Fix Toolbars" does to the icons causes an exception when VS2012 is starting and trying to load my toolbar, so it drops it and defaults to standard toolbars, yet after VS2012 is up and running it can then load the settings file to get my toolbar.
Anyway, I'm sticking with 0.8.1.1 for now since it works fine for me.

I loaded 0.9.1.1. After the restart of VS 2012, I got this dialog box:

    NiceVS

Cannot load icons profile from C:\Users\tom.sage\AppData\Local\Microsoft\VisualStudio\11.0\Extensions\izmyeaqz.uga\NiceVS.config: Could not find a part of the path 'C:\Users\tom.sage\AppData\Local\Microsoft\VisualStudio\11.0\Extensions\izmyeaqz.uga\NiceVS.config'.

    OK

Now when I run VS 2012, there is no change to the appearance.

    Thank you

This is a know issue :-(. It's related to a fact that Visual Studio changes location of an extension with a new version. Unless you see this message twice it's expected.
You should run "Fix Toolbars" (in Tools | Options | Environment / NiceVS) once more again (make sure that "Always reload toolbars" is checked before). After that toolbars should be persisted.
Just be aware that when "Always reload toolbars" is checked any manual changes you do via Tools | Customize will be lost after Visual Studio restart. Too preserve them you must click "Backup now" in Tools | Options | Environment / NiceVS.
