Wayne suggested we might have a system whereby annotations (dictionaries) might not (just) be enclosed by the element they refer to, but might also live elsewhere in a document, like the NOTES block. They would then be linked up with the element they refer to using a separate structure. This is much like the "mediator" element in an earlier iteration of the schema, which used XPath to make the connections, but it would be more limited in that it wouldn't use free form expressions, but combinations of IDs (e.g. characters ID + row ID) to build the reference.
