#123 mib2c should record options

I use mib2c about 2-3x a year and forget what I did
last time very easily. mib2c nicely starts the
generated files with:

But it'd be nice to have detail. Specifically, I can
never remember whether I picked:

1) tables where the list of rows is external to the
agent.

or

2\) tables where the list of rows is held by the

agent itself.

or, harder, which style of table iterator I used. If
mib2c could put my responses in the file, then 4-6
months from now when I need to create another MIB
handler, i could refer back to my old code and know
what to do.
