The Insert key does not update the "Overtype" prefs menu
checkbox. This has been true since at least 5.4.
Also, the Overtype mode does not carry over when a tab is
detached/moved, although the menu item is copied correctly.

Overtype menu item I was able to get into a state where I couldn't
raise the prefs menu with alt-p. I'm having trouble reproducing
this reliably, however.
I wouldn't call this a critical bug, but it should be corrected.
