Tried this (running NCO on a Windows machine):
ncks -H -d lat,32.,28. -d lon,245.5.,248.7. http://pcmdi7.llnl.gov/thredds/dodsC/cmip5_css02_data/cmip5/output1/MPI-M/MPI-ESM-MR/historical/mon/ocean/Omon/r1i1p1/tos/1/tos_Omon_MPI-ESM-MR_historical_r1i1p1_200001-200512.nc test.nc
I get a Error: nc_open unable to open file.
Not sure if I have a syntax error or if the issue is with the Thredds server?

I uploaded new Windows NCO 4.3.6 binaries, that fix part of that error.
However, that call fails because of ESG authorization. My suggestion is to contact LLNL to see if you can get some answers on how to deal with it
Yes the Thredds server does require authentication, which is sort of straight forward to obtain. http://home.badc.rl.ac.uk/mjuckes/esgfNcview/
NOAA Thredds server do not require authentication (but are currently down)
