NAPS2 saves as narrow PDF

Since a few months, NAPS2 saves the scan in a narrow PDF for me.
Sometimes the generation is OK, if I scan the document again, but usually it doesn't help.
When I save the document as a jpg, it has the correct aspect ratio. (so the scanner seems to produce a correct image)
When I rotate the page first, then the saved PDF is also deformed, but now very wide with low height.
My setup:
- Kyocera ECOSYS M5526cdw
- Profile: TWAIN driver, Use Native UI setting, Advanced setting: only Deskew scanned pages turned on (but also tried with it off), all compatibility settings turned off (also stretch or crop)
- Scan: original size A4, Feed method: 2sided (Binding Left/Right), Sending size A4, Black&White, 300dpi, Image quality Text.
