Upgrade NagiosQL to 3.4.0

Version 3.4.0 comes with a complete revision of the source code. Many pages have been renamed or deleted. Therefore it is not a good idea to extract NagiosQL 3.4.0 into an existing NagiosQL directory of an earlier version.

Recommended steps to upgrade:
1. Rename your old NagiosQL web directory
2. Extract version 3.4.0 to your document root
3. Rename the new 3.4.0 directory to your old NagiosQL directory name
4. Copy config/settings.php to the version 3.4.0 config/settings.php (this file does not exist there)
5. Run the installer URL (http://<your webserver="" url="">/<nagiosql root="">/install/index.php
