#4 Info about link submitter

It would be nice to add three fields to the links and
categories tables to track who added a link or category.

The three fields would be:
- Name - name of submitter
- Email - email address
- Comments - comments which won't show up on the public
pages (like reason for deletion/editing)

This extra info can be later on used to send notifications.

an email can/must be given by the submitter in v 0.7.beta3
