1. download the .zip file.
2. extract the contents to your DITA OT\plugins folder.
3. Start the OT cmd line using startcmd.bat.
4. Run "ant -f integrator.xml"

You can then run builds against your DITA maps using the "mypdf" transtype. I prefer to use the java command line. For example:
