Meaningful Use Transport Testing svn

Added constructor so test steps aren't null (now an empty List).

Added report type to report item.

Test Steps are now ordered by timestamp when retrieved.

Added hostname to xdr test step. Added two retrieval methods for getting record by hostname;

Adding "manual" to the list of criteria met possibilities in XDR logging.

Bug fix in escaping single quote character in SQL.

Bug fix in get by From Line.
