#13 Additional DB Creation Mechanisms

Add additional methods for creating the initial db for hosted situations without full access.

(1) Run (with confirmation) create/updatedb from
the administrative toolset (maybe via a
"database maintenance" option).

(2) Use PHP command line to generate SQL that can
be dumped into a SQL interpreter (assuming
adodb supports this).
