I missed my deadline.

Attached is my working patch, tests included.

I didn't implement the config file or environment variable checking. I 
don't really see them as being generally applicable. They're more 
specialized, and the behavior can be supplied easily enough by the 
author before calling ask().

The only thing I plan to add is the on_validate callback mentioned 
earlier. I'm still debating the best time/circumstances to invoke the 
callback.
