#10 changes needed to compile win32 version

This patch includes changes I had to make to compile
mod_cplusplus using Visual C++ .NET 2003. This patch is
against mod_cplusplus using boost.

strtoul is defined in apr as strtoul_is_not_portable...
so it breaks
cstdlib header from vc++, you need to redefine it any
time you include
files from Apache, otherwise you are not able to compile.

I replaced snprintf with boost::format, there's no
snprintf (there's
only _snprintf).
