Hey Jason, Check your SecAuditLogRelevantStatus setting - https://sourceforge.net/apps/mediawiki/mod-security/index.php?title=Reference_Manual#SecAuditLogRelevantStatus It should be set similar to what is shown in the Reference Manual wiki link so that it will log all transactions that generate both 5xx and 4xx (except 404) level status codes. -Ryan I have PHP in a FastCGI configuration on RHEL 5.5.
