Makefmt cannot find the input file (e.g., latex.ini). Try

   findtexmf latex.ini

This should give you something like

   C:\Program Files\MiKTeX 2.6\tex\latex\config\latex.ini

You should check your MiKTeX setup by running

   initexmf --report

Have you refreshed the file name database?
