The font is still not available on CTAN.
While I'm sure you have your reasons, it's not ovious to me, why the package has to exist on CTAN to be included with MikTeX. Could you elaborate?
I'd feel stupid to ask the lmmath authors to upload it to ctan without providing a good reason.
Also, my minimal example points out that the package unicode-math (which is available on in MikTeX) uses lmondern math fonts by default. Hence, it's a bit cumbersome, that \usepackage{unicode-math} would lead to an error.
Some package called lm-math is available on CTAN:

However, the file is called "lmodern-math.otf" instead of "lmmath-regular.otf". I'm not sure, whether it's the source of lmmath-regular.otf included with TexLive 2012.

It seems, they renamed the font (which basically renders unicode-math.sty broken, since it uses the wrong filename).
