Sorry for the delay, but I've been hammered with work.
I always forget the special command to toggle the folding, and it
seems that I've neglected to document it. Digging up an old email, here
are the relevant commands:
1. When showing a message, use "," to display everything.
2. Use middle mouse button over the field. Note that this only works if
your mouse is in the field (highlight will be green) but outside of
an email address (with yellow highlight). The field name is a good
place to mouse.
3. When composing a message, use "C-c C-t
(mh-letter-toggle-header-field-display)" when point is somewhere in
the header field.
Making this customizable seems like a good suggestion. Please make a
feature request for this (it may already be there ;-). I've made a note
to myself to add the information above to the manual.
