We prefer to have our Portal Items display in a new browser window. As a temporary fix, I modified Item:show_as_bullet() in order to add 'target="_BLANK"' to the <a href> tag. This gives us the functionality we desire for now.
What would be really nice is to make this a configurable feature. We envision this as an additional field on the Add Item form that would allow the user to declare whether he wanted the url displayed in a new window or not. (That's the new feature suggestion part of this.)
