#6 pyqt_memaid: filter & merge duplicate items

This rather large patch adds an option to make
pyqt_memaid check for duplicate questions and/or Q/A pairs
on "add new" and/or "edit items".

Rationale: I have about 2000 items of Japanese in my DB
and as the language contains a lot of homophones, I've
recently started to meet duplicate questions ever more
often. The feature should also be quite useful as a cleanup
for those doing frequent XML import/export.

As a bonus, the patch also adds a "delete current item"
action to the menu and the tool bar. This is handy for
cleaning up large databases "on-the-fly".

The changes probably pass the copyrightable works
threshold, so I added myself to the authors list to keep
the legal issues clear.

    Hi Jarno,

    I'm going through your patch, but I feel more inclined
    to put the options to for the duplicate checking directly
    under the 'settings' menu, rather than in the learning
    mode dialog, where it seems a bit out of place.

    OK?

    Sure, no objections, as long as the it doesn't get too
    cluttered.
