#45 Cloned mdb handle has references to original mdb

The mdb_clone_handle() function makes a deep copy of an
instance of an MdbHandle. However, the "entries" in the
catalog of an MDBHandle contain pointers to the
enclosing MDBHandle, which are not being updated to the
new MdbHandle after being copied.

This patch adds a single line which updates an entry's
MdbHandle to point to the cloned MdbHandle instance.
It is based on the CVS source dated 2005-12-30.
