[mdb-dev] mdb_alloc_handle / mdb_free_handle

At this point, I believe I can easily integrate the functionality of mdb_alloc_handle and
mdb_free_handle into mdb_open, mdb_clone_handle, and mdb_close.  I feel that
consolidating the functionality of these two layers into one would be a very good thing.
I wanted to clear this change through the list though.  Sound good?
