Example output line of `mdb-schema -N somenamespace foo.mde postgres`:

ALTER TABLE "somenamespace"."bar" ADD CONSTRAINT "somenamespace"."baz" FOREIGN KEY ...

Qualified constraint names are invalid in postgres (at least - haven't checked with other RDBMSs).
