I should also mention that the approach, of having an
integrated "workbook" environment with display equations,
executable code, graphics, text, links, etc. has the potential
to be "the documentation" -- at least the online version.

This general notion is supported in Wxmaxima, and other
front-ends.  Mathematica, Maple, and probably other systems
try to do this with varying levels of success.   If you have to produce
a written paper-based document, there have to be compromises.
