I think it's fixed now.  create_index was looking for no-bullet lines,
and maxima_singlepage has many of them unrelated to the ToC.

Modifed the script and Makefile to find the actual file with the ToC
and create the index from that file.

Then contents.hhc doesn't have the extra </ul> tags at the beginning.
I see that it also references the multipage html files in the links.
Hope that's ok.
