#7 Add comment to invitation and response

Users need the ability to attach a comment to prayer
invitations, as well as the intercessor's response. There
may be legitimate reasons to decline a requestor's
invitation and could easily be communicated with a short
note versus just have a cold rejection with no
explanation.

Also, the requestor may want to give a little rationale
behind requesting that person to pray for them.
