Here is the mplayer output on a working device with a working isight:

Lirc should definitely not be the problem. My guess is the

lines. So:
1) Are you using the "firmware patch" or a recent kernel that should work without firmware. Double-check if setup is correct?
2) Did you compile your kernel yourself? If so, did you forget something when configuring v4l?
Unfortunately I am no expert on v4l and the last thing I can suggest is to ask google for topics on that error message
