#2 Page headers and footers

Allow user to specify header and footer text to be put
on each page, specify font, text, justification. Also
maybe allow an option to include the page number in
either. Maybe the header could even default to the name
specified in the LDR file, the footer to (C) plus the
author in the LDR plus the current year.
