Oh, my system is mac mini intel with apple remote.  I've tried starting it with an absolute path to my .lircrc and defaulting to /etc/lirc/lircrc - neither works. ps aux tells me that lirc was started prior to irexec:

root      6226  0.0  0.0   2932   544 ?        Ss   10:45   0:00 /usr/sbin/lircd --driver=macmini
root      6236  0.0  0.0   1716   328 ?        Ss   10:45   0:00 irexec -d

if this makes a difference.  At this point irexec appears active, but it's unresponsive.  if I killall irexec then restart it:

root      6981  0.0  0.0   1716   332 ?        Ss   10:56   0:00 irexec -d

It works perfectly.
