Hi All,

Finally with some fixes and hacks CREATE AUDIO_OUTPUT_DEVICE is looking 
better.
Background seems to be working ok too. (if not for that other crash i 
was mentioning before having to do with the fact that we can't set the 
audio output type yet).
A lot of stuff still doesn't work. That includes destroying 
audio_output_devices, working with midi devices, mapping audio output 
devices to channels, detecting card specific parameter defaults and 
ranges etc, etc.
I'm going to try to fix some of those issues next weekend. Time is tight 
so bear with me.
