i seem to remember having written a script to generate these at some point
in time. it's relatively trivial perl IFF your html is relatively
consistent (that is, the complexity of the perl is inversely proportional
to the consistency of your html).  i'll have to dig up the scripts i wrote
for JLG oh-so-long ago. 

and it is my humble opinion that we should definitely have a TOC on every
page possible, even though it is "just" tips and tricks. it makes
referencing the document later far easier.
