It's a proprietary pinout cable and is connected through a destroyed modem. Basically Kiyoshi tore the PCB off of the modem connector and soldered this proprietary cable to all of the Dreamcast Parallel port pins on the connector. He then ran the cable into a PLL that used programmable logic to convert the Dreamcast parallel port to an IDE bus.
