Start of new CVS branch: (code not functional yet)
 Hosts now get their own generation counter.
 Hosts now have a pointer to a operation function table instead of to a
template structure.
 No hosts initialization is done anymore during lowlevel driver initialization.
 Hosts can be registered and deregistered at any time to allow switching to the
Linux 2.4 PCI interface and allow hot-plugging.
 Consequently, the initialized flag is not needed anymore.  Inaccessible hosts
have a negative generation number for that time.
