Re-arranged the console stuff to allow the CPU-specific code to register
its console when it wants to.  Machines with DZ11-based consoles now use
the driver in drivers/char/dz.c (mostly - still have to remove the DZ11
stuff in cpu_generic.c).

Cleaned up vax/lib/Makefile as well, and removed console stuff from tmp_init.c
