#5 Letter case lost

The case of all letters is lost during translation.

We could try to preserve the case of the original word or
use a capitalization flag in the destination dictionary.
We at least need to keep track of "first letter uppercase, rest
lowercase" and "all letters uppercase".
Note that single-letter words will be amiguous and should try to
use context which we don't yet have.
