Summary: Use std::string for SaxParser::on_get_entity() arg
Initial Comment:
This patch makes the SaxParser::on_get_entity() take a
std::string argument, to hide the fact that an xmlChar*
is used internally by the library and to be consistent
with the other SaxParser virtual functions.
