So far the maintainers of digiKam have always provided their patches, so yes, fixes from the KDE repository eventually get applied to mainstream. In my understanding libpgf in KDE is more like a branch than a fork. The branches are periodically synced, s.t. digiKam contains the most recent features and we have the latest patches. Disclaimer: I have not compared KDE and mainstream since digiKam moved to GIT.
