#214 NumberedPostingsPositionExtractor null exception

NumberedPostingsPositionExtractor: if a term is null (e.g. stems to null), this class throws a null exception.

Use of ## to insert space in a document appears arbitrary, perhaps a list of positions associated with each term would be a better method.
