Changed the fallback keyword search logic from OR (Union) to AND (Intersection).
This aligns the fallback behavior with the primary FTS search logic, fixing an inconsistency where fallback was overly broad.
Additionally, this improves performance by allowing the database to short-circuit non-matching rows earlier (fail-fast) and reduces the result set size.
Also refactored the query construction loop to be more efficient (single pass).
