#12 Adding switch lacks error checking

Adding a switch while leaving the switch IP and name
blank causes a "null" switch to be added to the
database, which causes all sorts of problems for other
parts of landb which check for null values. The add
switch code should check to be sure valid non-null
values were entered for the switch IP and name.
