Here is the latest EPT patchset.

Change from v3:
1. Build the identity mapping pagetable from kernel rather than userspace. The 
address was changed to 0xfffbc000ul
2. Fix the S/R and LM problem.
3. EPT enabled on 32pae host now.

The 2m page should also works, though test blocked by current hugetlb bug.
